What is your fertilizing regime?
Have you check your water kh level? Plants strive best at 4° but 3° seems ok too. My Yishun tap water has 2° kh so have to increase a bit.

I have this setup almost 2 years as to date, my plants don't seems to stay green and survive for 1 month or so
Can anyone give advice what I have not observe or something not right here?
The fishes are ok and have been staying on a year
CO2: 2-3 bps
using Ehiem 2215.
Clean the filter every half yearly or so
I don't see water condition abnormality.
Change water 30% every week.
Perhaps I should remove the ada soil altogether?

Kh, is this referring to the alkaline level?
glass indicator shows light green
Kh is the hardness of the water. Alkaline level is ph. The glass bulb indicator shows light green is to measure your co2 level. They are not the same.
Share your fertilizing dosing regime, so that we can see what is wrong with your setup.

How do I manage or identify KH level in my tank.

You can use any kh measuring kit to measure kh in your tank. The kit can be purchased in most lfs.
I'm not sure about the content in plant premium. But your aim will be to provide sufficient nutrients to your plants.
Nutrients means:
- trace minerals
- nitrogen
- phosphorus
- potassium
- iron
For my high light and co2 setup tank, I dose all these nutrients everyday. And perform 40% water changes weekly.
Many aquarists here will rather dose more nutrients than less as it's really hard to tell how much exactly the nutrients are sufficient for plants. So doing large weekly water changes help to reset the nutrients level in tank to avoid algae issue.

May I know the amount of driftwood affect kh level?
I have a bottle of nitrogen, never use it for a year. The green bottle is Azoo plant premium contain trace elements
I will began to dose them once a day. See how it goes.

Driftwood may not affect kh level but may affect ph. They will slowly release tannin and other organic acids that will slowly decrease ph making the water more acidic.

I think those plants look kinda like Dracaena braunii which is a terrestrial plant..? Usually sold as lucky bamboo
I used to have them with guppy. The leaf will Yellow and rot when submerged in water
