err help?

new to this so not really sure if i'm posting right..

but can someone help share with me why my carpet plant start to have patches of dead leaves?

also the leaves of the taller plant behind show signs of dying.

i fertilise with K and P on a weekly basis, around 3ml. My tank is 2ft and has CO2 injection.
