Fatworms are planarian. They stick to the glass and quite a number of fish FEARED them. Duno why.
Sometimes will also float on water surface upsidedown.
When infested in large numbers, they tend to garther around aerated area.
They are hardy and multiply quickly. One more thing, they fear SALT.
Planarians are known to spread TB to human. Dunno this worm species will.

When doing water changes, fiddling around the tank, etc, you come in contact with the tank water, how thoroughly do you clean up after that? Tap water enough?

From articles on the internet, planaria are said to be non-parasitic to fish. They will attack fish eggs though. A nuisance. I don't think they are that dangerous an organism. I am not sure they carry TB, but TB can be cured if treated properly.
To get rid of them for a while, use salt in tank, and use salt solution to soak the filter sponge.
