Anyone knows any crude/DIY method to lower down Water Hardness ?
And is there any cheap way where I can monitor Hardness constantly ?

you are living in singapore, right? so i don't think the water you are getting is very hard. what's the kh of your water?
the kh don't fluctuate if you don't do anything to your tank. testing the water after every water change will be sufficient. after a few water changes you will roughly be able to predict the water hardness in you tank.

High total hardness is not a bad thing. Read this from the rec.aquaria.freshwater.plants newsgroup.
