    which tank is this?
    more history will be needed

one hypothesis is that sulphur based bacteria (my guess akin to those on the bottom on the sea living on hydrothermal vents) is screwing up your tank biology

wow your fishload is pretty high! I donno what's wrong, but just a sidenote, why you use sodium bicarbonate, why not coral chips? cuz sodium bicarbonate will give sodium which you already are giving thru further addition of salt? or why not just add sodium bicarbonate and not salt? and I"ve no experience whatsoever but rem reading somewhere here that UV isn't meant to be on 24 hrs?

    not related to above.
    yesterday, bought some taiwan moss,
I sterilised it in peroxide + water mix.
then dumped it into the tank (without rinsing).

immediately all my yamatos started to drop like flies.

    about 10 died...

^^ so warning ^^ rinse before you add it in.

You probably have to open up of your dead fish and see if a parasite is living inside it. In my view, it is a bad case of internal bacteria. You can use interpret Internal Bacteria Medication on a ssample of fish to see if it works. Also, look out for worm like substances near the waster outlet of your fish.

Also, observe to see if the fish is feeding as usual. Normally, fatal disease fish do not feed at all.

May luck be with your fishes.
    Bless them with good health

when your fish are knocking off so rapidly
it's no longer time for a 20% change every week
try 30% daily until conditions improve
also, have you cleaned your filter? might be stuff rotting inside

generally, bacterial infections can be overcome by the fish without external help
in your case, i think it's beyond that
you could try using antibiotics in conjunction with the water changes
and remember to finish the whole course of antibiotics

    one dead angelfish...

From the tread about coppersafe to cure white spots, you said you used the three type of coppersafe solution.Suggest you stop using them(if you are)and see if any improvement to your situation.I've tried coppersafe once...and i swear never again.Less than 5min after i poured 2 capful of the coppersafe,my coolie loaches went wild and my other fishes(tetra and barb)were near the surface gasping for air.??????It jus my personal opinion about what might be causing the death of your fishes.I can't be sure though.Hope this might help.Heard about copper poisoning.Don't know if it apply to using of coppersafe(overdosage).Good luck!Hope to hear good news from you soon.

no, I'm not dosing any of those, right now, I'm using the MQ4 for internal parasites etc etc, in case it is.

last night found 1 dead apisto, think its the female cockatoo or something. and a dead whiptailed catfish (not dwarf royal farlowella)

the female cockatoo was standing on its head, scaled flared, ie: looks like dropsy, did a 30% water change, took out the fish into a small tank, died the next day.

my other tank = 1 dead peppered cory

will be cleaning up both filters on the weekends.

I'm also fighting some bacteria epidemic. Did a 50% water change for 4 days and then a 80% change on the fifth day. Shack man!! Fishes seems livelier, but still got casualties.
I have dwarf cichlids in my tanks! Do you?

ok: changed 2 buckets of water, and cleaned up all my filters.
    the female agazzizi is dead..

found my male cockatoo floating upside down, labored breathing. moved him into a small container..

seems like all the critters from tohs are dead, weak immune system? other wild apistos in my tank are carriers for some disease?
gill flukes? hmm: might do an autopsy...
