Looks serious. Could be disease. My Staurogyne is disintegrating similarly and upsetting the tank balance.

I've tried to bump up the nutrients but it's been going on for the last 5 months.

not sure lay..
my previous batch of glosso also like this...
then it turn brown and disappeared..
thought my rummy nose attack but now this tank got no fishes still like that...
sigh...

My nana also having the same problem (see my thread).

If all your other plants also having same problem, there must be something wrong with your water.

Hope some expert can come in and help .
