Perhap u have over dose or it is due to other reasons?
Due to white spots in one of my harlequin, i stupidly dose some aquarium salt into my tank of 20 cardinal, 20 harlequin, 8 otos, 10 yamatoes, 10 malayan, 2 clown loach, 1 golden algae eater, 2 big SAE.
A few days later, 3 x oto passed away.

Did you dissolve the salt before adding into the tank? Fishes do flinch and dart around when they get in contact with undissolved salt. In the case of catfishes (e.g. otos), the reaction can be fatal.
FYI, I've treated white spots with salt 2x so far and no oto died on me (I had 6 otos in my 2ft tank then)

Personally, to play safe, I dissolve all solids in water before pouring evenly over the surface of the tank (unless otherwise stated on the packaging). Try to avoid pouring directly on top of fishes. Mixing into topping up water is a good oppourtunity as its a comparatively larger volume of water.
