Bad case of tail fin rot that the tail portion is severely affected, irreversible damage on the tail section.
If it was detected early, you can still save the tail portion and regrow the tail fin with medication.

Most likely is due to poor water quality.

From what I can see, it is likely to be Plistophora hyphessobryconis, a sporozoan parasite that usually infects fish like these Tetras, hence the common name, Neon Tetra Disease. Because it weakens the fish, the fins will be affected by rot as a secondary infection. I do not think that there is a known cure for this infection so the only course of action is to cull the afflicted fish before it spreads throughout the tank.

Oh thanks, I got rid if it coz it died shortly after. But it's been 3 days, the rest if the cardinals seems ok.. Touch wood..

I've got a 5 ft tank stocked with tetras. It some times it's cause they fight n the open wounds start to lead to tail rot which worsens and they die :/ doesn't happened often but it does

Just hope that none of them get a whitish patch on parts of their body. I have seen whole tanks of tetras afflicted with this NTD at some LFS.

Just net out one more with a white patch on head... Changed 50% water... Hopefully no more casualties
