What Insect is this?

Hi, can anyone help to identify this insect in my shrimp tank? It's a 1 feet no CO2 and filter tank with only java moss and cherry shrimps and 1 oto as occupants. This insect appeared shortly after the tank was set up 5 months. Disappeared for a while and recently made a come back. What causes this and how can I get rid of this awful looking "insect". Thanks.
