CO2 seems to check out but do recheck it for consistency as it can burn folks both new and old.

    Routine for your tank
    1)Trim off infected plants
    2)Clean the filter
    3)Do a large 70% water change
    4)Review the CO2
5)Add back 1 teaspoon of KNO3 after the water change
6)Add 15-20ml of TMG after water change
7)Add 1/8 teaspoon of KH2PO4 after water change (very much cheaper than seachem phosphate. Uptake of KNO3 would slow down if PO4 is limited.)

If plants are growing very well after that and the plant mass increase by several folds, you can try 2x a week. But your tank should be fine with once a week as long as you keep up on the fish feeding.
