What nutrient deficiency is this? Keeps happening to my plants

That usually indicates a lack of potassium and/or Co2. For those affected leaves, its best to trim them away, as the holes will not recover back anyways. Dose more fertilizers and increase your Co2 (as safely as you can) to improve the plant growth.

I have the same problem too...
I have some plants having brown holes too.... Other then lack of co2,can it happen without proper soil??

Well, most plants can take in nutrients from the water column too, so if your soil or substrate doesn't have enough nutrients, just dose more fertilizers into the water column. Most of the time, brown holes appearing on leaves are due to nutrient deficiency.
