What is the best suggestion? to leave it and monitor? or to change the whole soil?

If not all plants affected, then should not be substrate problem. If all plants affected, then it's probably substrate or the entire biotope problem.

If you suspect fertilizers, then just stop using the fert and run active carbon filtration to remove the suspect heavy metals inside the fertilizers. Can also add some water conditioners to remove toxic metals etc.
