It's Anubias species. There are many varieties. Just do a image search in google.
I have been seeing this plant "nana" being mentioned in this forum. Does anyone have a picture of it? Thanks.

It's full name is Anubias Barteri var. Nana.
Anubias Barteri is
Note the slightly different leaf shapes and I'd say there's a difference in size too... var. Nana is smaller than the normal version.
Is this the one that is always sold with a small DW or rock?
Yeap, both of them are usually sold on driftwood blocks
