Don't worry about it, it only shows that they are doing well. You can cut the plants below the offshoot roots and replant them again, but check on the cuttingtechnique as many people warned that there is a certain method for each plant.

I have planted some stem plants recently in my tank..
I noticed many "roots" along the whole stem... why is that?? am i supposed to cut the roots that is coming out? or should i cut stem and plant??
Is this due to water condition? light condition? or the fact these plants were in land cultivation, but i just put in water?

It's normal, people just cut off the roots that are showing that's all. All part of stemmed plant maintainence.
