I just bought a 15cm cube azoo tank. One neon tetra got suckup the tube and was mashed up. Now it has 5 tetra swimming happily.

You mentioned a tetra got suck up the tube? What filter are you using?
Tetras are pretty big to be suck up a Hang On Filter.
