We can't really tell without a photo or a better description (e.g. size of holes, etc). I'll hazard a few possibilities.
Could be nutrient deficiency. Click here for a chart of symptoms.
Other possibility is something is eating it. Any new fishes recently? Or new plants may have introduced snails.
