Identify first, and attempt to treat the sick fish in a separate hospital tank. If that fails, I am prepared to see it die. Better 1 dead fish than an entire tank down with an epidemic.
Always underdose slightly when it comes to aquarium medication. Overdosing can have catastrophic consequences. General purpose medications are terrible. I remember buying those cheap fish medication with some funny illustrations on the bottle wrapper. Aquori or something, I don't remember. After that was some other brand, probably locally made.
If the disease is untreatable, or usually has a very slim chance of recovery, I let nature take its course. Diseases like dropsy, internal bacteria infections, fungus (especially mouth fungus), I usually do not bother to treat, because they are difficult to combat.
