One of the main reason would be your nitrate level ( No3 ) , it should be at 10 ppm or below. I suffer some shrimp death when it hit 20 ppm.

Increase your CO2 to about 3bps and turn on half hr before lights on and off the same time as your lights off, dose Nitrate (about 10ppm), Phosphate (2ppm) and Flourish (follow instructions on label). After one week change half tank of water. Keep lights on for 10hrs.

His nitrate ( NO3 ) already 100 ppm which is already super high, stiil need dose? I thought nitrate too high will be harmful, am i wrong?

First thing first i think your gh abit too high.2nd you should do water change to your tank around 20% to bring down the nitrate.Dont forget to use dechlorinated water also

I suggest 20% water change every 2 days for a week. Followed by weekly 20% water change.

Your GH is too high, nitrate is off the scales for keeping shrimps, also there is some nitrite in your water which is poisonous to fauna. Meanwhile, I don't think you need to dose anything until your tank stabilize. If you need more info, feel free to PM me.

Assuming your test kit is correct, your NO3 is very high. Regular water change should help reduce the NO3.

By the way, I can't really see from the first picture, but you seem to bury the windelov fern. If you do, take it out, do not bury fern rhizome, otherwise it will die.
