Excess Phosphatecauses the leaves to turn Black or Brown and to die.

Hi guys, I just set up a simple low tech tank recently (1.5 weeks ago) and filled it with green cambomba and Narrow leaf Sag.
-Tank Size is 2.5 Ft.IMG_3910.jpg(click for bigger pic)
-The current fish population in the tank is about 15.
-I have NO injected CO2 and but instead use CO2 tablets and Flora Pride Liquid fertilizer.
-Light is a Day Light + Blue light flourescent 55 w that I use a timer to turn on for about 7 to 8 hours a day.
-Using a mixture of ordinary Gravel and fertilizer gravel (not too sure the proper name for this but it can be seen in attached pic, it's red and pallet like )

My current issue is that of late, the Narrow leaf sag in the tank have started to turn yellow from the tip of their leaf and also brown in one or 2 stalks!

What can I do about this?? Is it due to a lack of Iron or an excess of Phosphate? Does'nt the liquid fertilizer help with this already? How can there be an excess of Phosphate when my tank is only 1.5 weeks old? Confused..
Appreciate any help or suggestion from you guys! Thank You!

Thank you. But I have just changed half of the water and the tank is less than 3 weeks old. Could it be that my solid fertilizer is low grade and thus adding to the phosphate?
