Water change is too little at 10% a week. Due to heavy feeding to your CRS. No. of hours your lights are too long (10 hours). Also, fertilisers used is none. This resulted in unbalance. Lighting Intensity : 39w x 4 t5 is too strong. It is 156Watts!!!

Lastly how much tank water you have got?
