Unhealthy root of unknown plant

Hi,

I am new to planted tank so pardon my ignorance on the protocol to seek for advice.

Its a small 3G tank. HBO filter. CO2 tablets dosage every 4days. 11W light on for 6hrs daily.

How could I improve the poor condition of my plant's root. I am assuming its the root. I suspect it could be the stem that is not receiving enough light.

What is the name of this plant?

I was having problem getting this plant identified not too long ago here. However, what you are seeing is quite normal. Now after 3 months my plants are still doing okay. It will develop some new roots
